This course focuses on adult health, introduces the history of disease management, identifies key concepts of chronic disease management and explores chronic disease management models.

Detailed Description of Conduct of Course

 

This is an online, distance learning, course organized around student-centered learning modules, experiential activities, films/videos, lecture, case studies, guest speakers, and investigation of published literature. This is not a self-paced course, students are required to complete weekly course content. Students are required to attend synchronous sessions throughout the semester.

Student Learning Outcomes

 

After successfully completing this course, students will be able to:

1. Discuss the concept of chronicity.

2. Identify the key elements of comprehensive disease management systems.

3. Analyze the impact of disease management on healthcare systems.

4. Evaluate the impact of disease management on individuals, families and communities.

5. Examine interprofessional roles in chronic disease management.

Assessment Measures

 

Assessment measures may include a range of strategies such as student presentations, discussion boards, quizzes, exams, papers, virtual trainings training, case-studies, and online survey tools.
